Southern Surgical Arts offers a secure login to our electronic medical record so that you may complete your new patient forms in the convenience and privacy of your home.  The Online Patient Portal saves you time before you appointment, and is a safe method of keeping your information current at our office.  And, it allows Southern Surgical Arts to Go Green!

Just click on the computer icon below, and  you will be directed to a new window where you will be asked for your security code.  Please call our office and we will email the security code to you.  Then, you will be able to create your own username and password.

Please click on the New Patient Information Pack link to print your Information Pack at home.  This packet is meant to inform you of the cosmetic services, both surgical and non-surgical, offered at Southern Surgical Arts.  We hope to guide you through the beginning stages of becoming a new patient of Dr. Nease, and/or any of his staff members.  It will also contain information about our three locations, the appointment and treatment process, as well as general financial information about our practice.  This is your first step in understanding more about our services and polices.

New Patient Forms - Print at Home Option

The New Patient Forms are available for you here on our website if you choose not to use the Online Patient Portal.  Please know that the Online Patient Portal is the preferred method of completing your Patient Health History because you can update your information at any time as often as you like. And, the Online Patient Portal allows us to Go Green by saving paper! 

If you are unable to access the Online Patient Portal, the please use the link below to download the New Patient Forms and bring to your consultation.  Completing these forms before your appointment will save you time before your appointment.  Please click on the link below to open a pdf version of our New Patient Forms.

Pre-Operation Instruction

The Pre-Operation Instruction is solely for cosmetic surgery patients of Carey Nease, MD, at Southern Surgical Arts before surgery at either the AAAHC accredited surgery center in Calhoun or a local hospital. It is not intended to be medical advice for any other persons and should never take the place of advice from your personal physician.  This information is given at the pre-op appointment for patients of Dr. Nease at Southern Surgical Arts. If you have any questions or concerns, please call our office to speak with one of the nursing staff.  If you have a medical emergency, please call 911 now or go to your local emergency room.
